Often people are lucky enough to plan for a life time and live exactly according to the plan. however that is such a rare thing. More often than not, people tend to start out a certain way and end up in completely different corners of the world doing completely different things than planned.&lt;br /&gt;&lt;br /&gt;To find yourself in a  foreign situation is one that needs at least some amount of adjusting. One could certainly make the most of the situation. All situations in life have both some good and some bad in them. I often tend to see more than my fair share and try to reflect on weather this was a good thing or bad.&lt;br /&gt;&lt;br /&gt;On my seemingly usual day, on the Adolescent psychiatry unit, i came across a young teenage lady who was admitted for suicidal behavior from a placement facility. If anyone spends 5 mins in these units, you would know that it is a common occurance for us to see kids from one facility be dropped off in an other for several reasons. This particular individual who also suffers from borderline traits, has unfortunately  stuck pieces of  plastic comb in her left forearm, thereby allowing it to become an abscess. although this abscess was drained and one part of the debris removed by a surgeon on an outpatient basis, this girl continued to complain of pain, and the inflammation didnt subside. she repeatedly complained of leftover debris, and one can actually feel a small hard nodule in her left forearm above the recent sutures.&lt;br /&gt;&lt;br /&gt;However as a resident in psychiatry one is not at liberty to order testing at ones discretion. when i did ask my attending about the need for atleast a doppler to r/o leftover foreign objects, the response was "offer tylenol" and make an outpatient appointment.&lt;br /&gt;&lt;br /&gt;this occaisionally bothers me,  as a psychiatrist, one goes through medical school, then to residency, one therefore should not loose the skills necessary for primary care, and one should infact make it a point to practice primary care. i often wonder why the field of psychiatry has to remain so far away and elusive from Medicine. Perhaps the Inclusion of atleast or some part of medicine will make it more acceptable in the mainstream society.
